I think it would help if you told us a little about why you are doing this, in what context it will be used and for what purpose!

I did something similar to this for a college dissertation type thing. People do like to know why your doing the survey. I also found the results were highly contradictory so think about how you word your questons even if you feel sure about them for example I asked "is is ever ok to clip a parrots wings.?" Most people ticked "its never ok" but then further on when I gave multiple choice answers to "in what circumstances fo you think clipping is acceptable" ticked no end of boxes despite still having a "never" box to tick. It made it very hard to get my results together- but it helped me find bad points and improvements to write for my evaluation.

1. I have 2 exotic birds
2. one male :gcc: aged 7 and one female :corella: aged 9.
3.myself, my son, his dad and my neighbors have regular contact with my birds.
4.they are kept indoors during winter but they spend time outside when weather allows.
5.both are cage free unless they are sleeping.
6. neither are clipped.
7. it's natural for birds to fly they benefit physically/mentally from the exercise of flight.
8.yes, my cousin and aunt both have birds and they are clipped.
9. no.
10. I think wing clipping benefits the person more than the bird but I think most people think they're doing it for their bird out of fear of what they think will harm their birds if they are able to fly like escape or injury.
