you may want to try this:

when he's hungry (i.e right before breakfast), place him on top of his cage and target him to fly to your hand, while you are standing at a short distance away from the cage. click and reward, and IMMEDIATELY place him right back on top of the cage. then repeat. when you see that he's comfortable doing this, you can slowly start increasing the amount of time he sits on your hand before being placed back on top of his cage. also, you can also start slowly increasing the distance between yourself and the cage, so he has to gradually fly farther.

you can also try taking him for a short "walk" around the room while feeding him treats, and immediately replace him back into or on top of his cage. that way, he'll think, hey this is not so bad, I get yummy treats for leaving my cage, and i get to come back after all. slowly you can start increasing the duration of the walks, and the interval between your treats, as you notice he's getting used to things.
