Avian Sun Bulb Spectral Output Showing UV

As I mentioned on another thread, I contacted AvianSun and Featherbrite asking for documentation of the spectral output of their bulbs. With AvianSun's permission here is a version of the original they sent me which they said was OK for me to repost (the original one they sent me had numerical units on the output on a log scale and wasn't nearly as pretty). Here it is. The UV-B is just there, and I'm guessing the UV is coming from fluorescence in the coating, which is why it "wears out" after a few months. But that's my speculation. The bulb does put out UV though.

Do these come with a bulb? I was recommended the floor lamp by the avian dept manager at the shop where I've purchased my Senegal. It is $45, but the box says it requires a "lamp" sold separately. So is the base just a base?

How much do the individual bulbs cost and how long do the seem to last?

The bulbs are fairly expensive, I think I paid $25ish, and they only last 6 months. The bulb will continue to light for a good long time, but the UV output goes down over time. However, you don't need a special base, any lamp that supports normal incandescent and compact flourescent bulbs will do the trick.
