Scratching/rubbing is normal, they do it to clean their beaks. The rest I don't know about but deformities could be from old injury or nutritional deficiency, the beak changing colour could be age related, disease or deficiency. Please get him a check up with an avian vet to be on the safe side.

You should really see an avian vet before adding supplements to his diet. Self-diagnosing can sometimes aggravate the problem.

Does he eat vegetables? Parrots should be eating more vegetables than fruit, because fruit is very high in water and sugar. Try veggies like sweet potato, kale, carrots, cucumber, etc. You may have to try preparing them different ways (whole, mashed, chopped in big or little pieces, cold, or warm) to see how he will eat it. Nuts and seeds should only make up a small percent of the diet; most people here use them only as training rewards because they are so high in fat and low in other nutrients.
